Prescription Information
A prescription is required for the purchase of all continuous positive airway pressure (CPAP) devices. We set your machine by doctor's order and must set it to the settings indicated on your prescription. The CPAP PRO® has a prescription sign-up page and can be filled out on-line after purchase for instant review.
Below are the requirements for a complete prescription. Having all the required information will speed up your order fulfillment.
Checklist for submitting your prescription
- You can send it via toll free fax at (888) 771-2082, by email at prescriptions@nomask.com, or using the form below.
- We will automatically match up your prescription with your order.
- Prescriptions must be issued by a doctor licensed to practice medicine in the United States.
- Make sure that the prescribed pressure setting is clearly visible on the prescription.
- Make sure the contact information of your physician is clearly indicated.
- We only need your prescription with pressure setting and NOT the entire sleep study report.

Upload Your Prescription
We require a valid prescription with the pressure setting(s). You can fax your prescription to us toll free at (888) 771-2082, email it to prescriptions@nomask.com, or upload your prescription file using the tool below.
Prescription Uploading Checklist
- We only need your prescription with pressure setting and NOT the entire sleep study report.
- If you have already placed your order online with us, please reference your order number on your prescription. We will match up your prescription with your order.
- If you have not placed your order yet but plan to do so online, you can still upload your prescription, but please make sure the name on the prescription matches the name on the order.
- Make sure that the prescribed pressure setting is clearly visible on the prescription.

Presciption Uploading Instructions
- Scan your documents into a .jpg, .gif or .pdf file. Please limit the file size to 5MB (megabytes).
- Click the "Browse" button on this page.
- Browse through your computers files, locate and double click on the image file containing your prescription information.
- Enter your full name, Order ID Number (on your email receipt), daytime phone number, and email address.
- Click the "Send Prescription" button on this page.
Please note: the upload may take several minutes depending on the size of your file and the speed of your Internet connection.
After uploading the file, you will be taken to a webpage verifying that your prescription uploaded successfully.
